FORM OF SECOND NOTICE UNDER CLAUSE (a) OF SECTION 23

No.____________ Notice was given to all registered practitioners whose names are included in the register maintained under Gujarat Medical Council Act, 1967 ( other than those who have been registered provivionally ) vide No _________________
dated _________ published in the Guajrat Government Gazette , Part dated _______ to sent their applications to the undersigned for continuance of their names on the register.Separate notice with application of their were also despatched to the registered practitioners at their addresses shown in the register.

Further noitce is now given to all practitioners who have not so far returned the application forms duly completed to the undersigned, to return them within the time specified in the notice sent to them.Applications sent after that date will be accepted only on payment of fee ofrupees two alongwith their applictions.

Separate futher notice alongwith another application form is also being sent by registered post to each practitioner whose application is not received by the undersigned within the time allowed, specifying the futher period during which the application will be received only if accompained by a fee of rupees two . The names of all practitioners who fail sent their application along with the fee of rupees two durning the futher period allowed will be removed from the register as defaullters.
