CPS Additional Qualification Permanent Registration

Candidate has to create their user account on www.gmconline.co.in for registration process.
All particulars shall be filled by the applicant only on online portal www.gmconline.co.in
Candidate has to bring Printout of physically signed Application form along with below mentioned documents in original & one Xerox copy
- Print copy of Appointment details stating Payment Transaction ID number
- Photo ID Proof (Any of Following)
  - Aadhar Card
  - Driving License
  - Passport
  - Pan card
- MBBS Permanent Registration Certificate issued by Gujarat Medical Council (Xerox)
- CPS Provisional Registration Certificate Issued by Gujarat Medical Council (Original)
- CPS Permanent Degree Certificate
- Bonafied Certificate issued by Institute
- Government of Gujarat Rural Completion Certificate
- Maharashtra Medical Council Registration Certificate & NOC for CPS Qualification
Total Payment of Rs. 15800/- is to be made through online payment Method
Certificate will be sent by Registered Post on the given address submitted
